Film Release Form (Interview)
By signing below, I agree to the following terms regarding the Program titled “The Strings of Life”:
FILM RELEASE: I grant to the Producers, their licensees, agents, successors and assigns the right in perpetuity, to use in and in connection with the Program (in any manner it deems appropriate, and without limitation), by whatever means exhibited and/or advertised:
Types of Media (as used in connection with the Program): Videos, recordings of my voice, my actual and/or fictitious name, and biographical material about me as used in the Program and/or in connection with the promotion of the Program.
Modification and Scope of Use: I agree that the Program may be edited and otherwise altered at the sole discretion of the Producers and used for any and all broadcasting, social media, online streaming, promotional and/or exhibition purposes in any manner or media.
Grant of Rights: My participation in the Program is granted freely and voluntarily, in perpetuity and irrevocably. I understand that I have no rights to the Program or any monetary benefits derived therefrom. I enter into this agreement with the full understanding that my acknowledgement will be screen credit or screen ID. I further understand that all musical compositions in the Program will remain the property of the original writer(s)/publisher(s).
I represent that I have the right, capacity, and authority to enter into this agreement (the “Agreement”) and that my performance and the rights I have granted in this Agreement will not conflict with or violate any other agreement or understanding I have with any other person or entity.
